The last two forms on our account have some how been spoofed with my own name and my daughers name. I'm the owner of this account and would not be registering for a high school field hockey clinic. I'm 44 years old and my daughter has already been registered once before.
What is going on here?
I need help AS
AP as we will have several forms completed in the next few days here and need to know they are legit.

Well if that's the case then someone from you or someone who knows you tried your form out. We at JotForm team doesn't do such thing, that's strictly not allowed. If we are going to make tests with the users form, we usually ask permissions and enter values related to testing like "JotForm Support Test" or etc... Also, I don't think that's a spam since spams or bots usually submits random letters or generated characters. So, I highly suspect that someone from your team or someone from your place did it.
My suggestion is to track the IP who submitted that data, you can check that on your form submissions page.
