We have correctly integrated a form to google spreadsheets. We have not edited the form since the creatiion of the spreadsheet, but the spreadsheet is no longer updating when new submissions are made. Is there a reason for this?

Have you edited your Google spreadsheet? Please remove the current integration and re-integrate your form with google spreadsheet.
Please let us know if that resolves your issue.

That doesn't seem to solve the problem. We have removed/reintegrated numberous times now. Each time we reintegrate it creates a new sheet with all the previous submissions to date. However, each time a new form is submitted AFTER we have reintegrated, the new submission is not automatically pushed into the google spreadsheet.
At this time, we have made NO edits to the google spreadsheet (even though eventually we would like to have that functionality). Any suggestions?
*Also, we TOTALLY love your form builder, and we plan on upgrading to your paid services once we can resolve these issues :)

Request you to please let us know the form ID in question so that we can have a look. We will test it and if required will forward your issue to our development team.

The Form name is DD Reservation Form

This is due to the starting numbers on your form labels e.g. 32" HD Flatscreen. Note that there's some limitations on the API which possibly breaks the integration when a number is positioned on the first place. I would suggest to move these after the text labels instead. Example:
I have cloned your form here and did this, it works after integrating the form to GSpreadsheet.
