Users try submitting form from iPad and receive a blank screen when hitting submit. The information they submitted does not show up in jotform and their whole submission is lost. This has been reproduced several times. The form in question is the on the "one time purchase" tab.
A speedy response would be appreciated. Thanks!

We are sorry for the inconveniences caused.
This is not a common problem so we will need to investigate it further. Can we add a Free product to the form for testing purpose? Also, do you have information about users' iOS versions on their iPads?

Sure, you can add a free product. iOS version is 6.0 (10A403)

Unfortunately, free product is not going through since your form is in a live Stripe mode. I have cloned the form to my account configured the Stripe integration with my test account, then submitted the form using iPad with iOS6 through browserstack.com, I was not able to reproduce any issues.
However, sometimes there might be conflicts with JS form and other scripts on the site. Can you please try to use the iFrames to see if that makes any differences? I hope it can solve the problem.
