Form calculation widget on the not worked until the width is made zero

Terrible instructions. I followed them 10 times and the field would not add.

Then I looked at properties, and removed the width to zero (number of characters that could be added), and then it worked.

Sorry about that. The width doesn't have any impact on calculating fields with the width of the field. The form calculation widget is default set to 20 characters of width. I have cloned your JotForm to my account and added a new Form calculation widget. The form calculation seems to be working normally by adding the fields with operators.
