I have a form on a private employee portal that works 80% of the time but then I have employees whos forms fill out and refuse to submit. This is an error message some employees receive. What is the reason for this?

Will it be possible for you to give access to your portal site so we could check the embed coding? If this is not possible, can you please try reembedding the form or check whether the form domain url that was used in the embed script is jotform.com, if it is not, please change it to jotform.com

Looks like I def embedded this a really long time ago. The code I currently have is:
<iframe style="width: 100%; height: 1087px; border: none;" src="//form.jotform.com/form/13384324502" height="240" width="320" frameborder="0" scrolling="no"></iframe>
Would simply going in and using the wordpress embed rather thant his iframe, help?

Yes its seems you are still using the old embed code. You can try using the Jotform WordPress plugin if you are using Wordpress platform or copy the new iframe embed code then switch to text mode in your wordpress website page and paste it there.
