Is JotForm right for my office?

    I've provided a link to a form that our patients can open, fill out, print themselves, then bring in to our office. Notice that there are spots for intialing and signatures. How can JotForms make this type of form work for me? I'm not opposed to recreating the form online and the signature can still be done when they get to our office. Ideally, they would be able to fill out the form online, submit it, then come to their appointment where we would have their form printed for them and ready for their signature. Hope this makes sense. In the past we have used an asp secure server-side program to accomplish this using this exact form in the link with a "submit" button enabled at the bottom of the last page. I've disabled the submit button for now because the form has to be sent securely for privacy reasons.

    We currently have an e-signature feature that you can use for what you are looking for.
