Sorry but I can't access your site here. Can't even access your domain aenys.com. I know it's UP according to "isup.me" but the website you provided isn't available on my side. The page keeps loading and nothing is displayed. I would suggest to review it on your end and get back to us again.

We'll be waiting your reply. Thank you!

Hi Elton -- I have no idea why the website would not be accessible to you.
Is there another JotForm tech who can address my issue, located somewhere they can access
    my site?

Hi there, I am able to view your form perfectly on my end. I am currently viewing it in Chrome and also FireFox with no issue.

Are you using the latest version of Chrome or FireFox? Could you please provide us with further information on what versions you are not seeing this in so that we can troubleshoot this more?

My colleague and I discussed your issue some more and are wondering where or how you are not seeing this form additionally? Since he and another colleague of mine on our support team are unable to view your website this leads to a possible probability that your Website Host may be restricting certain IP's that are not local or that may need to be specified. This might be related to the issue you are having since it is known that some Website Hosts out there block this making them location specific and that may explain why your form is not seen.

If you continue having issues please let us know and we will gladly help look into the issue more.

    Hello Kade,

I never said I could not see the form.

I meant the form was not functioning. The forms cannot be filled out,
nor do the drop-downs work. Sorry for any lack of clarity on my end.

I was able to see your form here http://aenys.com/erase3.html

But just like as you described, the form is like disabled as I cannot click on any of the fields to fill out.

I suspect this is due to script conflicts with the jquery scripts in your website page template as I can see a them on the html page source code

Can you please try re-embedding  the form minus the 'script' part. Like this..

    <iframe id="JotFormIFrame" onload="window.parent.scrollTo(0,0)" allowtransparency="true" src="//form.jotformpro.com/form/31685559876979" frameborder="0" style="width:100%; height:1414px; border:none;" scrolling="no"></iframe>

Noticed I omitted the script part of the embed code. Make sure to clear all the old codes first and re-embed clean.

Please try this and inform us if it does not make any difference.

Thank you for your efforts, Jonathan, but it still doesn't work (I cleared all previous code out, and inserted your code):
http://aenys.com/erase3.html  It still works in IE, but not Chrome or Firefox.

I just inspected your webpage and I see you have now inserted the iframe version of the form

However, I think the problem is that your page is missing a very important doctype declaration. Please replace the current one by  this one

    <!DOCTYPE html PUBLIC "-//W3C//DTD XHTML 1.0 Transitional//EN" "http://www.w3.org/TR/xhtml1/DTD/xhtml1-transitional.dtd">

Let us know if the issue persist

Thank you anyway, Jeanette, but I did as you suggested, and the problem persists.

Can you please remove this orphan tag </form>

If the form doesn't work still, I would recommend you to delete the <table> tags as well as   <td>  , <tr>   and the opening and closing <div> tags

In other words, since you have embedded the iFrame code, you don't need to insert this code within tables nor div tags
