Can you please confirm if your issue is about not receiving email notification from your form?
Your form email recipient is this email@example.com. Can you please confirm if this is correct? This is different from the email address you used in your account and according to the mail logs, the submissions notification was successfully delivered to this email address. I have also make a test submission to your form, you should be able to received it in that email address.
