My form is on this url: http://videotranscriptionservice.com/customer-order-form/ I have made 3 different submission emails to try and correct this problem but the choices don't seem to come up as an option to have them in the submission email. I used some advice from a previous post in which is says to get the field ID and create the proper coding. Now, I'm receiving 2 emails, one seems like it may be working but the other is giving a Question & Answer box which means I've done something wrong! I don't know why the 3 choices have never appeared in the first place. Please can someone help? I need to get the gist of this and where I'm going wrong as I am hoping to use JotForm on many future websites. Thanks.

Sorry for the inconvenience that may have caused. With regards to the issue, have you tried already re-creating your form's notification? If not, request you to please re-create your form's notification and delete/remove current notification if that fixed the issue or not.
However, If the said issue still persist, do let us know.

Hi. I removed all the notifications and created a new one from scratch....It's working! Thanks so much for your help. I have my faith back now!! :)
