Embedded form requires login

    Hi,

    I used the embed code for my webpage but it doesn't seem to work.  You an see our user icon on the upper right corner AND if I sign out of Jotform then people only see the log-in page.

    As per checking, your form is set to only be available for anyone signed in to JotForm with domain eyesonyoupc.com.

    If you do not want this restriction, then please set your form to be Public.
