Uploading a PDF is not completing to convert

    Good morning,

    I'm trying to upload a PDF and it's taking an exceptionally long time to upload.  Please advise if something is incorrect.

    Happy to help you today.

    I would like to inform you, if you can clear your browser cache, close it and reopen to see if that works for you, also try under incognito window and see if that does the trick.

    If you are still finding the same issue then kindly confirm the PDF file size or may be send it to us and we can try it from our end.

    Hi there

    Thanks for your reply.  I did what you suggested - cleared my browser cache, uploaded again, and used incognito window.  This has not done the trick.

    The PDF is 458KB. I would like to accept your offer to upload it for me. As it is a PDF how do I upload it as you only accept images?

    I have tried downloading the PDF and importing to my account and able to replicate the issue. The issue seems to be the table or blocked cells in the table and unable to convert the PDF form.

    Since the PDF form is having only a table, it is easy to create form using JotForm by adding Input Table field to the form.

    Hi There

    I have further investigated and the form needs to work in the following way.

    If a Field is selected i.e Always Tired, then in the columns; numbers 1,2,4,5 and 8 need to be "selected" and count as 1 each. So column 1 = 1 and column 2 = 1 for example. 

    As a person completes the survey and selects different fields that are applicable each tick needs to calculate at the bottom.

    So what I would need assistance with is the following.

    If Always Tired is selected, all the tick boxes that are white need to be selected and count for 1 point. Then once the survey is completed all the ticks in column 1 need to be tallied at the bottom. 

    Secondly it would be preferred if once Always Tired is selected that all the available tick boxes in the row are automatically ticked and calculated. 

    How would I go about creating the above?

    Hi there,
    Many thanks for all your assistance with this form issue.

    I have tried everything suggested to make this work and am still unable to get this right.

    Here is the issues:
    * My client wants people to fill in a 55 question lifestyle analysis questionnaire with 9 columns that will require input on her website.
    * Certain cells within these columns will need to be checked according to the users response to the questions of YES and NO.
    * The cells in that row where the answer is YES need to be checked. These cells should have no colour in them.
    * The cells will remain unchecked if the answer is NO. These cells should be grey in colour.
    * We need to count the number of checked cells and total this. The total must be hidden from the person completing the questionnaire.
    * The completed form with totals needs to be sent to the owner of the website with the users contact details.

    How do I do this? Please assist.
