Add checkboxes only on Imported PDF

    Hello,


    I uploaded a PDF to JotForm that already has questions with checkboxes. When I try to add checkbox widgets, it wants me to add text. I don't want to add text, I just want to add checkboxes that people can check. How do I do that? Thanks.

    You should add either a Single or Multiple Choice field to the web form then map it to the PDF. Here's an example:

    Thank you. I added those boxes and now my form has 2 sets of the same questions. Kind of annoyed. Please advise. I am going to try to delete the first set of questions or I will just start over.
