Table survey question is not displaying correctly on card design

    I would love to use the card design layout for my survey and I am unable to display correctly the table questions?? 


    Can this be solved or would I have to use the classic form instead of the card one to show the table?? 


    Please help. the card design is much nicer and I do not want to display all survey questions at once.

    This is most likely caused by the HTML code added to the field elements.

    Note that HTML code is not supported on the form builder, I would recommend you to remove this code and set up the options as plain text. 

    Also, responses posted on this ticket are sent to the email address configured on your JotForm account, I'm afraid that it's not currently possible to send responses to a different email.
