I have tried 3 different email addresses and 2 different domains. I am receiving the TEST notification email and the administrative registration confirmations, but the notification to the end user fails to arrive.Page URL:

Make sure that all of your form's email alerts (on each form) are set this way
-SENDER EMAIL : firstname.lastname@example.org

Have you checked your spam folder?
