Google Sheets: Why isn't the data sending?

Regarding form "Conversions - Lookalike (1%) - Medical Mobiles"

Syncing with google sheets is not working properly ... leads stopped going to google sheets automatically and I had to remove integration and add it again to have all leads sent to google sheets..
