Form Section

Hello:

The section of my form (Dependent Information) is not showing up on the website page. What should I do to resolve this problem? Thanks.

Please try to duplicate the field and delete the original one. 

Please note that removing a field from the form will also remove all related data to that field. This solution is only applicable when you don't have any submissions yet.

It seems to be a JS conflict with the page where you have embedded the form, since the field is showing up within the original URL:

I suggest you try a different embedding method (refer to this guide for further information: https://www.jotform.com/help/34-Embedding-a-Form-to-a-Web-Page) or replace the widget by another field.
