how to create an intake form to send email

    Hi,

    I'll be creating a medical intake form for my office. My intention is to get their information like name, contact address, email, phone, etc. I want to email them these forms so that they can fill.

    You can create patient intake forms by adding particular fields that will help you to collect certain information about your patients. When you go to form builder and start creating a form, you can click on Add Form Element and find the fields to add to your form. These fields include full name, email, address, phone number, and more. Other elements can be used for collecting patient health information. 

    While you can create a form from scratch, you can always visit our form templates gallery and take a closer look at available forms. You can find relevant forms in the healthcare forms category. 

    Regarding how to send an email, you can always share your forms via email so that your patients can fill them out before the visit. Once you are done with the form, you need to go Publish and copy the email provided. You can share the link with your patients and they fill it out. Other sharing options are via social media like Facebook and Twitter or via QR code.
