I was able to create and run the visual report on the form you have provided which can be found on this link. Can you please provide us more specific details on what you mean you are not able to run a visual report as I might have miss something?

The report only runs two questions. What about the rest of the questions?
Your link didn't work. Were all the questions answered in your report?

The visual report should be similar as shown below

Yes, I deleted it because the report only covered two questions. I need the
report to cover all the questions. The above report link still only covers
    two questions, no good.
Plus, from JotForm, I'm unable to save this form.

"The visual report should be similar as shown below" does not appear in my
email, just a blank image that bleeds off the page.

The reason that you might not see all the questions in your Visual Report at first is because you need to adjust the " Entry Limit " if you have more than one question per field. Now depending on the chart you are using you may need additional charts to see them all but the problem with that and a longer form is that you will be limited on what can be seen otherwise it may not appear at all or even show up squashed-looking.

If you are unable to save your Visual Report or Form then it's a good idea to clear your form cache then also clear your browser cache. Now log back in and try it again.

As for the Report via your Email can you please try reattaching the image or linking it. I am unable to see the problem either at the moment since a screenshot did not display on your last response. You can use our Image Tool or ImageShack if you wish.

    Your reply was useless, because:
    - There is only one question per field (see
    http://www.jotform.com/csv/31813881946059)
    - You don't explain adjusting the "Entry Limit"
    - My cache was cleared
    - it wasn't me who attached an image, IT WAS YOU ALL
How do I run a report on some of the questions within my survey?

I apologize for causing more confusion to you. Let me clarify some things in that case then.

    - There is only one question per field (see
    https://www.jotform.com/csv/31813881946059)

On your form I can actually see that there are several questions per each field depending on the question though only one will be displayed depending on the choice. An exception to that is the Textboxes.

    - You don't explain adjusting the "Entry Limit"

My fault for not explaining this more so here's an update on that. You can adjust the Entry Limit via the Visual Report Configuration (Grey Cogwheel). Since it's limited to only 5 Entries this means that not everything will show if there are more than 5. So per say if you have 7 questions but only 5 of those got chosen and the last 2 did not then those wouldn't show up because of this.

    Example:

    (1) Edit your Visual Report

    (2) Now select the chart and click on the configuration

    (3) A New window will pop-up allowing you to edit it. Now you can adjust the Entry Limit

    Result:

The difference will jump from A to B to show comparison on this. As you can see, the chart got bigger to show what was originally not there since some of the questions to be answered were not shown at first.

    (A)

    (B)

    - it wasn't me who attached an image, IT WAS YOU ALL

However, the only thing this still does not explain is why the form is " bleeding off your page " that you mentioned and that is what slightly confused me when you reiterated " Visual Report should be similar as shown as below ". I reread it again and realized now that you were actually speaking about my colleague's image so that was my fault for my misunderstanding about it.

It shouldn't be going off the page and it shouldn't be blank either unless you've deleted the report which would cause report to be blank since there would be no data on the visual report. This sounds more like a browser issue. May we know what OS and Browser/Browser Version you are viewing this problem in? So that we can do further testing to check on this for you. This is what I was wanting to know more about when I was referring to uploading or linking an Image.

    - How do I run a report on some of the questions within my survey?

The visual chart unfortunately is limited on certain aspects such as combining all the data since the results would most likely become a catastrophic mess if done that way. I think this is why we have other options to simplify it better so that it is less confusing. Also this only works for surveying checkbox, radio button, etc whereas a Textbox would not show up. You can adjust the " Chart Data " to only show the questions you want for certain answers based on specifed fields and you will need a chart per each field to show the data that is collected. You can view this difference in the image below. A pie chart will show the data in a nicer format instead of a bar since the bar might overlap if the text is too long. So because of this if you want to display. So if you are missing anything it is because either someone has not answered that particular answer, you haven't created a chart for the field, or you need to change the Entry Limit in the assigned chart so that the answer appears.
