how can i go back and undo something on a form

Hi There, I was working on the Teaching Preference Form and I had a large paragraph at the top of the form explaining how to complete etc. and this seems to have disappeared? how can I get this back please?

Hi @Laurasheppard, thank you for reaching out to us. 
You can make use of the revert to the revision history tool. Simply click on the clock icon beside "All changes saved" to launch the revision history tool.
