I need to change my username

    My account keeps getting locked out. Please consider this an urgent issue.

    Please kindly note that accounts enabled with HIPAA Compliance are being lockout due to multiple failed login attempts.

    Unfortunately, you cannot change the username when HIPAA is already activated on the account. You have to disable HIPAA, wait for our backend team to remove HIPAA on the account, send us an update so we can proceed on your request. 

    We also need the new username that you would like to be. To check the availability of the username, open a new browser and go to our signup page (https://www.jotform.com/signup). 

    Note that we do not recommend disabling HIPAA if your forms are currently active.
