Line up patient name with DOB

Don't understand your recent response to our question about lining up the patient name with DOB? From Baystreet Pediatrics. When we preview it still doesn't line up for our update patient information form.  Is there any chance you could include twinsdoc4@gmail.com on your response?  Thanks, look forward to hearing from you.
