Form has not enabled again

Form has not enabled after the 1st of the month?!! Says that we have 0 submissions but still won’t let customers fill them out??

I checked your account and have noticed that you have received five submissions so far this month. 

By any chance, are you referring to this username, ty2nat, or a different one?
