    My form was working just fine and then I went back on my website and when I click on the link it says "Form Not Found." I went head and cloned the form for a new link and it continues to have the same error. What should I do?

    It seems your account therapeuticinnovations was suspended due to your form 201346567170049   ollecting social security number.

    Please be aware that online form builders, including Jotform are being used for identity theft in the past.
     We go to great lengths to prevent this. That is why SSNs are considered as a Phishing activity as stated in our Terms

     Therefore, when you do request SSN, our automated phishing detector will flag the form(s)  and suspend the account or this will be manually performed by our Staff, if your account is on paid status, you should get a notification about it.

    To lift the suspension in your account, we had to delete the SSN field in your form.

    In addition, if your purpose is not to collect these data from the general public, but from your customers, then you need to comply with two things to be able to add the SSN field back to your account:
    1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

    3. Otherwise, you must enable form encryption on your form if you need to gather critical personal information
