Your forms do not work

    I created a form (<script type="text/javascript" src=""></script>). When I uploaded to my site and tested it, the form data I got back in my email was the data the was in the ORIGINAL form you used as a sample. Instead of INVESTOR SURVEY, I got your sample SPORTS CLUB SURVEY, as follows:

    What do you currently think about the KMCC Sports Lounge? No
    How often have you eaten at the KMCC Sports Lounge? 
    What do you think of the food selection currently offered by the KMCC Sports Lounge? 
    How would you rate the value of food to the price? Free Mentoring
    If you could add to the menu what particular food items would you like to see on the menu? 
    How long are you willing to wait for food? Yes
    If the Sports Lounge is changed into a European Pub would you like to see a German Style Bar? 
    Which is more important for a European Style Pub, the drink selection or food selection? 51-65
    What type of food would you like to see in a European Style Pub? 
    What type of atmosphere would you like to see in a European Style Pub? 
    What other suggestions would you have for a European style Pub? 
    If the KMCC Sports Lounge was converted into a European Style Pub would you be more likely to patronize the facility? I'd have to think about it
    We are looking for a new name for a European Style Pub, any suggestions? No

    I need a form that sends the responses to MY survey questions.

    Hi, Sorry for the inconvenience caused. I request you to delete your notification and create a new Notification Email.

    This will Solve the Problem.

    To Delete your Notification:

    1. Goto Email Alerts under Setup & Embed tab in Form Builder.

    2. Select your Notification.


    3. Click Delete Email Button


    To Create A New Notification:


    1. Click Add New Email

    Did not help - once i deleted as you suggested, then clicked ADD EMAIL, I got a form that I am unable to change. But I STILL do not think you are understanding the problem! I AM GETTING THE EMAILS. I AM JUST NOT GETTING THE CORRECT DATA. I AM NOT GETTING THE DATA MY FORM ASKS FOR.

    You aren't getting the correct data simply because your email field tags aren't correct or you might have swapped them. My colleagues advise should help. When you recreate your email notification, it would reset all the field tags to the appropriate one and that should fixed it. If you'd like to customize your notification email, please make sure not to changed any of the field tags.

    I would suggest to review your email notification. The embedded form here has the right tags so it should now work properly.
