How can I create a full Health Insurance Application with multiple applicants?

I am not sure if I understand your query correctly. As I check your email address, I notice that there are 3 forms created on a guest account. Do you want to create a form to receive submissions from multiple applicants? If so, you may receive multiple submissions to the same form created in your JotForm account. The number of monthly submissions vary by the subscription plan. 

Please try creating a JotForm account using your email address so that the created forms in the guest account shall be automatically moved to your account. You may edit the forms as per your requirement or use a template to clone it to your account.
