Need help with the mapping of the fields in PDF Form

    I need the please circle portion removed and then under the contact numbers,

    I need a spot to type in the number and indicate if cell, home or work.

    Hello!

    - I need the please circle portion removed

    I see the "please circle" potion on your PDF under the Contact numbers (below the second fields). Please note that removing PDF content is not possible with JotForm. You'll have to delete these fields from your PDF using offline options (can be easily deleted by the person who created this PDF). Once deleted, simply upload the Form again in Jotform

    - under the contact numbers, I need a spot to type in the number and indicate if cell, home or work. 

    Fields, typing-in the Phone Numbers can be easily achieved by first creating Phone fields (on the Web Form) and then mapping those fields onto the required places/spots of your PDF.

    Similarly, for choosing between the "Cell, Home and Phone", Single option fields or Dropdown fields can be first implemented on the Web Form. Then these values can be mapped onto your PDF Form.

    We can do this for you if you want us to. Also, I've prepared a short demonstration that shows how to create fields and map the same onto the PDF.
