Documents in forms?

    Hi Support, 

    I have a form I would like potential clients to fill out based on medical injuries after they complete, I would like them to sign my custom agreement & HIPPA form.

    Step 1 - Medical Information
    Step 2 - Sign & Initial my custom agreement and HIPPA form via Docusign widget

    Can I combine these all in one form somehow?

    How do I accomplish this with JotForms?

    I would need the client's information filled on the blank fields of my agreement.  Is there a widget for this... I have looked at the videos and searched but could find anything.  I would prefer not to have to recreate my whole agreement.

    Hello @DiBacco,

    Thank you for contacting our support team.

    While it is not possible to combine two pre-existing forms, you most certainly can add the correct widgets too your form to have all of this work together.

    You can keep the current from that you have now and then simply add an Initials Widget to capture your patient's initials and a DocuSign Widget, to capture your patient's signatures. 

    Then, if you would like to add a paragraph[h section where your patients can fill in the banks, then you can add the Passage-Test widget to your form.

    All of these widgets can be added straight into your form so that you do not have to re-create anything. All you will need to do is enter your credentials for your DocuSign account
