My first form attempt is stressful.

I created a form titled Resume Worksheet.  I am unable to get the dates to reflect the same look when the form is published although I have checked to ensure that all specifications are the same for each.  The Start and End Date fields appear to be in two different formats.  I just want to get one look or the other, not two different looks.

I checked your form and indeed it is quite weird what the date field is doing. This seems to be happening for forms created using a template or have applied a theme.

I tried to simply toggled on/off the Lite Mode setting on one of your date field just in case it did not properly saved at first, and it did the trick. Kindly do so on all your date fields and I suggest taking 1-3 seconds or more per action to ensure that the changes are saved.
