I want to create a form for my company staff to use

It is not clear what product we need for our company and the costs.

We need a form completed and signed everyday by up to 40 employees.

We would like them to complete on their phone or desktop.

I realize this is possible but not sure what the cost is to the company and what each employee has do to get it installed on their phone.

You may create a form in your JotForm account and receive any number of submissions to it. However, the number of monthly submissions received are limited by the subscription plan of the account. Since you mentioned that you need your employees to sign everyday which we expect about 1000 submissions in a month. So, here are the two subscriptions Bronze or Silver that you may consider. 

With the Bronze subscription plan, you can receive up to 1000 submissions in total in a month while Silver plan allows up to 10000 submissions per month. Based on the number of submissions that you are expecting to receive, you may upgrade your account. You may check our pricing page for comparing the subscription plans. 

And, it is not required for the employees to pay anything to submit the forms in your account. They can access the form using the direct URL and submit it on a PC or on a mobile device.
