Form not working with Firefox

I have checked the link that you have provided and I am unable to find any issues within the form when using firefox. Could you please tell us if the issue has been fixed or provide what the exact issue is currently?

Privacy badger programm  is the problem it simply does not load the page with the JOT form....after taking it off it worked again
