Conditional logic does not copy the field value

    when patients submit a form and i have the option on to populate the name on all forms it does not show up? i will attach the form i get returned back

    I have checked your form, and the name on the first page is copied to the whole form:

    right thats what i wanted, now when i get the email on the PDF the name is left off on all the pages, i must of clicked something because it was doing it before

    I think the PDF document needs to be reset, so I would suggest creating a new PDF document:

    Choose the new PDF document in the email attachments.
