How to create a golf T-Time Form

Hi,

I am curious if there is a form or a widget where you can add dates/times available, but once they are claimed, that date/time no longer is available for future customers, in order to avoid double booking?
Basically, I am looking for a form where golfers can register for T-Times and once the T-Time is claimed, it will no longer show as available.
