My jotform account is suspended

    My jotform account suspended please help.

    And login not work please help.

    Suspension reason #1:

    Your account got suspended because two of your forms are collecting ID or government issued ID without the two necessary widgets. We will have to remove it for compliance. If this was a government-issued ID it needs to be protected by two mandatory widgets:

    1.) Add the Terms & Conditions Widget(or the Short Scrollable Terms Widget) for your users to read and agree.

    2.) Add a Signature Widget so your users can sign and agree to provide such sensitive information.

    The widgets will apply for ID number collection or for ID upload collection. This will make you the sole responsible for collection and handling. I have now deleted the offending fields from your form and activated your account. Please make sure to add the widgets before adding the fields back in order for the system to recognize a valid collection.

    Suspension reason #2:

    Unfortunately, you cannot use a textbox to collect credit card information/bank details as it is against our terms. Your form 201590981532456 was suspended due to the collection of the bank account number.
