Help with input tables

Please note: I do not know how to create CSS code.  I can insert the provided code.

On page 4

I would like all three input tables to be at the left margin and the same size.

In table 3, Pedigree, I would like the three input columns to be the same size (large enough to show the dog's name and titles).

Help is greatly appreciated!
