Conditions not working on Embeded website

My jotform has conditions set on dropdowns to direct queries. For example, if "Clergy" is chosen, a Clergy dropdown is revealed. There are three "types" of contacts in the initial dropdown - Clergy, Staff, Functional Area.

When I view the form from jotform directly, it all works perfectly. But when the form is displayed on Square Space, the secondary dropdown doesn't appear, thus the user can't select the person.

The screencap shows the jotform screen with the fields involved (i.e., in Contact Area, if the user chooses Clergy, the Clergy dropdown appears, choosing Staff causes the Staff dropdown to appear, and so on.

In addition, the form on SS is not presenting the fields in the correct order. For example, the submit button is on the middle of the form.

Is there a way to make this work?

I have checked your form, and it looks like everything looks as it should. 

The submit button is on the bottom, and additional fields show depending on the Contact area selection:

Just to be safe, I have emptied the form cache.

Can you please check it, and let us know if the problem is still present?
