Was testing a credit card authorization form and now I can't log into my account. Sometihng about phishing?

Hello Justin! On your "01) BWC Credit Card Auth KY" Form, you're collecting Credit Card information from your users (in plain text fields) which is strictly prohibited on JotForm (screenshot of the fields attached).
Our system detects it as a Phishing activity.
Phishing. You agree that JotForm Inc. may terminate your JotForm Service immediately if a form’s purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

There are other ways to collect Credit Card information from your users. You can use one of our payment apps to gain your user's consent to bill them later.

We can enable your Form and your account if you agree to delete the above mentioned fields from your Form.
Let us know if you're okay with it. We'll delete these fields and re-enable your account.
