Radio buttons missing in the pdf editor

Hi.  I just received your response.  Yes, the radio buttons show up on the builder, but they do not show up on the PDF.  This Contact form will not allow me to include the PDF to show you.

Could you please provide a bit more information on your goal? Would you like to be able to download filled PDF files based on submitted form data (responses converted into PDFs), or to be able to download a blank PDF form? We have two types of PDFs in the PDF editor.
