How can I get this form off the ground and functioning in our office?

    Hi JotForm Team,

    I designed a form.  It is everything we want and more.  We have been approved for HIPAA compliance and we want to start using it.  BUT, I'm having trouble getting it from development to execution.  I'm sure its my lack of knowledge in the stuff that comes second nature to you.  Please help.  My team is looking at other options like intakeQ and I'm not against it but I've put so much time into this that I want it to work.  We feel we will have to find a convenient way to email the form to our patients as getting them to go to our site to fill it out is like finding an oasis in a desert.  We're finding navigating around your site to find the url klunky and perhaps there is an easier way we haven't discovered.  Also, we want patients to be able to securely sign consent and send pics of insurance cards and I don't know that we have that done correctly.

    I have checked your JotForm and it seems to be working fine. However, there are not many fields that are required on the form. This may enable the user to skip the fields to fill on the form. Please check again and make the fields as required fields as per your requirement. Also, I notice that the Take Photo widget being used to for the insurance cards. While this widget also works, if the form is being accessed from a PC it may not be possible to upload the file. Instead, you may consider using a File upload field on the form that also allows to take a photo when the form is accessed using a mobile device.
