How can we allow up to twelve individuals to complete the first part of the form to send it to the treasurer?

I want to allow up to twelve individuals to complete the first part of the form. From there it would go to the Treasurer (also a Trustee) and he/she, if approved, would get two of the other Trustees to sign it off. Then back to the Treasurer to complete the process.

Can this be done?
