How to customize the form's URL?

We would like to have the ability to have our own branding and use our own URL instead of seeing the jotform URL. Is enterprise our only option for just those abilities.

Yes, if you are referring to using your own domain, this feature is only available to Enterprise users: How-to-Use-Custom-Domains-with-JotForm-Enterprise

As for regular plans, you can create a Custom URL or customize the URL name. You can add your own form name at the end of the URL along with your Jotform Username: How-to-Create-a-Custom-URL-of-a-Form
