My account has been suspended! - Please help

    I believe my account has been wrongfully suspended. Could I please speak to someone who might be able to help me.

    This is a time-sensitive enquiry as I rely on Jotform to operate my business.

    Hello Gabriel,

    Your account was suspended since the form was collecting sensitive information such as National insurance number and also upload of IDs.



    If your purpose is not to collect these data from the general public, but from your own customers, then you need to comply with two things:

    1. Add the Terms & Conditions Widget (or the Short Scrollable Terms Widget) for your users to read and agree.

    2. Add a Signature Widget so your users can sign and agree to provide such sensitive information.

    To comply with our Terms, I have removed these fields from your form and activated the account. You can add them back along with the above widgets.
