My account has been suspended

    Hello,

    I just received an email that my account has been suspended and I’m wondering how to re-active it?

    Hi, your account has been suspended since your form is asking for SSN and drivers license, which is considered sensitive information. Asking for sensitive information violates our terms: https://www.jotform.com/terms/

    "Phishing. You agree that JotForm Inc. may terminate your JotForm Service immediately if a form’s purpose is found to be deceptively obtaining, for example: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately."

    It has also been found suspicious, you appear to be providing internet services. Please email reviewers@jotform.com with thread #2397830 as your subject. Make sure to provide the URL of the website where your form will be used, and proof of affiliation to a legit institution.
