There are four conditions set to send out my auto responder to 4 different email addresses:

Is this working fine previously? I just checked the emails used in your form conditions and they're all not in the bounce list so it should be fine. Can you please try to make a test with your form and see if you are able to received it's notifications now?

I did a test, it seems to be sending only to the last condition.

There are 2 questions in the form that request email addresses. There are conditions set to Auto respond to those emails. But they are not getting the auto response.

I looked at your form and it appears that the problem is resulting from a misconfiguration in the Email Notifications. I checked this by changing and using some of my own email addresses but I didn't recieve anything when I normally should without a doubt.
It appears that the problem is resulting due to a misconfiguration between your Email Alerts and the Conditons. I noticed that when I added in a New Autoresponder I was able to start receiving some of the notifications. However, before that...nothing was coming through so I think this was because you had either didn't actually have the Autoresponder or the connection between the two is missing.
I will need some more time to try to figure out what is happening here. You may want to clone your form and then play around with this so that you dont mess up the original until you get it right since sometimes conditions can be slightly touchy if done incorrectly.
