Form has been set for Multiple submissions, but is not working. can anyone help with this issue? thanks . . .

The Unique Submission settings may have been enabled in your form. Kindly verify and disable it again in your form preferences if it is indeed turned on:

Using a form template which i modified, and expanding form layout on iWeb, resolve both issues.
