How to activate suspend account?

My account have been suspend. How to activate? And why it suspend?

Your account is suspended by our system because the form you created is violating several of our Terms. 

You added copyrighted logos and you are collecting IDs, passport numbers and banking info without the user's consent. 

In order to reactivate your account, kindly provide us the following information:

1. The URL where your forms are used.

2. Your website or proof of affiliation to a legitimate institution.

3. Share with us a document that will prove that you're authorized to use the said logos.

We also need you to change your registered email to a corporate one.

Send these proofs to reviewers@jotform.com with Thread #2403071 as your subject. We will let you know about the status of your account after a thorough review of your proofs.
