Adding multiple lines to the question?

Am I able to add another line to a question? Or even a description in the single page form layout? I want to the question to look like the attached screenshot but when I hit enter, it won't go to a new line?

I got it to work in the single page format but need it on the other format.

I have checked your screenshot and it seem to be a single choice field.

The multi-line question is only available on the Card Form Layout

and can be found in your Basic elements

It is not possible to add a single choice field on a multi-line question format.
