I cannot embed the form to my website

I have the form complete in form builder but when I click preview form the number of nights & number of rooms with the price does not display and when I try to embed the form on my web site I only get the link. My old form was displayed as a widget on one of my website pages. My Link does work but I still cannot get the the room selection portion to display

You need to disable the Show or Hide conditions to see the Payment field properly. As of this moment, you need to select the Cash option to show the Payment field.
