Any file type will do, please try uploading a logo to your form to see the results. You can also test the PNG file type logo/images if your current logo file type is not working.

And to allow multiple selections in products, please see my screencast below for reference.

    Hi
I made the changes you showed me and was able to select more than one item
at a time but I don't want to have payment made when the request form is
submitted. I want to collect the credit card information when I contact the
customer back for the conformation of their reservation. Is there a way to
accomplish this without going through a gateway?

Unfortunately, collecting credit card information outside PCI-certified payment fields is not allowed. This is against our Terms of Use and might cause your account to be suspended.

Now, this one is optional but you can take advantage of the feature called "Authorization Only" which allows you to collect payments manually at a later time. Here is a more detailed guide on how to do this: How-to-Enable-Payment-Authorization. However, this doesn't store the complete card details as well.

I have an Authorize. Net account and use them for all my credit card
transactions. The problem with collecting the card info with the form
submission is inventory. I have a limited number of rooms available at any
    given time.
    Any recommendations?

If that would be the case, you can manually collect the credit card details from your customers using any other means, such as email.

Due to PCI-compliance, we strictly implement using payment integrations. Since you mentioned you are using Authorize.net to process credit card transactions, you can create a separate form for that and process payments there using the said integration. Here's a guide on how to integrate Authorize.net with your form: https://www.jotform.com/authorize-net/

Is there an error message displayed when you try the form?

Can you please tell us what you experienced when you added the Authorize.Net integration to the form? I checked your form and you deleted it so I was not able to check the payment integration.

I did not want to charge people making a reservation request, I was using
the form thinking it was great for the people filling it out to see what
the cost was for the room they were requesting the reservation for. I did
not know it had to be linked to a gateway. I spent a lot of time trying to
create a form I did not want.
The form I now have does everything I need
Thank you for all your help
