I log on with our user name and password and click my forms....and it is blank. We should have one form out there with dozens or maybe even hundreds of applications received.

Hello, please try clearing your form cache then log out and clear your browser cache. Now log back in and go to My Forms and see if it's still blank. Please let us know if everything is okay after that or not?
I looked on my end and I can see your account is only showing 2 forms in the My Forms page.
