Adding any widgets add signature

every time I drag a widget into my form, its just the signature one. Seems to be a glitch going on.

Can you please let us know does this happens in all forms? If not, please let us know on what form the problem is occurring? 

Also, please let us know what widget are you trying to add so we can try to replicate the problem.
